De functie
Supports, educates, and assists patients and their families, serving as a link to doctors and supportive services.
Key role in implementing multidisciplinary care for Anticoagulation and warfarin maintenance.
Assists in securing prescriptions and supplies, coordinating care for warfarin management.
Participates in telephone triage and contributes to evidence-based nursing practice to prevent patient harm.
Demonstrates teamwork within an interdisciplinary care team and fosters a culture of respect.
Cardiac experience and anticoagulation/warfarin management are preferred; requires RN license, BSN, and BLS.
