The role
Support the mission to improve community health by developing, implementing, and evaluating programs that advance population health and reduce health disparities.
Lead program development, implementation, and evaluation with a focus on health equity and addressing the cultural and linguistic needs of a diverse membership.
Compile and analyze qualitative and quantitative data and prepare concise narrative reports for staff, members, and providers.
Create materials, handouts, newsletters, presentations, and website content for staff, members, and provider audiences.
Develop and maintain partnerships with community organizations, vendors, and provider partners; participate in community coalitions and facilitate meetings and trainings.
Plan and conduct focus groups and surveys to obtain member and provider input; work Monday–Friday 8:00 am–5:00 pm in a hybrid setting with 1–2 remote days.
