The role
Provide patient-centered physical therapy in home health settings for patients from birth to old age, focusing on functional improvement and mobility.
Deliver evidence-based therapeutic interventions, adhering to established standards and optimizing patient outcomes.
Generate timely, accurate documentation to verify medical necessity and support compliant billing and authorizations.
Collaborate with patients, families, and the care team to coordinate care and education across the home environment.
Maintain professional development, stay current with best practices, and manage time to meet productivity expectations.
Qualifications include a Bachelor's degree in Physical Therapy with PT licensure; a Master's degree in Physical Therapy is preferred.
