The role
Deliver diagnostic and therapeutic physical therapy to home health patients as part of the plan of care, including assessment and treatment planning.
Evaluate functional status using tests and measures, apply therapeutic activities to reduce pain and improve mobility.
Monitor patient response, document findings, and communicate with the physician and care team to revise the plan of care as needed.
Coordinate care through interdisciplinary collaboration, attend care conferences, and make referrals when appropriate.
Assess the home environment for safety, train patients and caregivers in home exercise programs and the use of adaptive devices.
Maintain accurate patient care documentation, ensure timely updates, and participate in quality improvement initiatives.
