The role
The Care Coordinator Medical Interpreter bridges communication between healthcare providers and patients with limited English proficiency by delivering accurate interpretation and translation of complex medical information during visits.
Supports both in-person and virtual appointments, translating medical conditions, treatment options, insurance details, and follow-up instructions.
Coordinates post-appointment care by facilitating referrals, developing multidisciplinary care plans, and setting self-management goals.
Acts as a system navigator for managed care members, addressing benefits access, coverage questions, and barriers to care while protecting patient confidentiality.
Participates in interdisciplinary team meetings, tracks care outcomes, and reports program measures to optimize intervention effectiveness.
Requires a two-year degree and recognized medical interpreter certification or LanguageLine IST, with fluency in English and at least one required foreign language (e.g., Mandarin, Cantonese, Vietnamese, or Spanish); valid driver's license for travel.
