The role
The Clinical Pharmacist is employed under the NHS England Additional Roles Reimbursement Scheme to support primary care teams by conducting medication reviews, providing prescribing advice, managing medication queries, and supporting disease management. The role involves medication reconciliation, safety monitoring, and independent prescribing where qualified, within a multidisciplinary setting. The pharmacist collaborates closely with GPs, nurses, and healthcare professionals across multiple practices, contributing to service delivery priorities and policy implementation. They perform clinical governance activities, participate in audit and quality improvement initiatives, and engage in ongoing professional development. A registered pharmacist with GPhC with primary care experience and strong communication skills is required.
