Unit Clinical Care Coordinator

Oversee admission, progression, care coordination, and discharge planning within the nursing unit to ensure efficient patient flow. Lead daily cross-functional huddles with nurses, physicians, social workers and ancillary staff to align discharge goals and resolve barriers. Manage documentation and progression/discharge data in care coordination systems, escalating unresolved issues to leadership. Coordinate with bed planning and utilization review to ensure regulatory alignment and timely patient progression. Develop and execute comprehensive care plans, coordinating with clinicians and ancillary services to optimize length of stay. Communicate progression and discharge plans to patients or proxies, and ensure preprocedural requirements are met.
