The role
The Primary Care Nurse III (RN) delivers high-quality ambulatory nursing care for pediatric and/or adult patients, supporting daily clinic operations.
Responsibilities include nursing assessments, medication administration, immunizations, POCT, test result review, and EMR documentation.
Leads care coordination and participates in population health initiatives to address gaps and improve health outcomes.
Performs telephonic, electronic, and in-person triage and educates patients and families using teach-back methods.
May travel to multiple clinic sites to train staff and review processes; collaborates with providers and staff to ensure patient safety and quality care.
Requirements include a Bachelor's degree in Nursing, an active NYS RN license, BLS, 4+ years of related experience, and strong communication and teamwork skills.
