The role
Provides direct nursing care in home settings, focusing on wound care, ostomy management, and pressure injury prevention for patients and families.
Develops and implements a comprehensive wound/ostomy care plan based on medical, social, and environmental assessments.
Monitors patient responses to treatment with the physician and adjusts plans as needed to optimize outcomes.
Assesses learning needs and provides patient and family education, counseling, and health guidance using appropriate materials.
Leads and participates in committees, educates staff on best practices, and develops policies to support evidence-based wound care.
Maintains up-to-date knowledge of resources and ensures coordination of care across disciplines, with additional related duties as assigned.
