District Nurse Team Leader
Lead and coordinate a district nursing team to deliver safe, evidence-based care to housebound patients in the community. Manage a caseload, delegate visits, and ensure timely, high-quality clinical documentation and care planning. Provide day-to-day leadership, coaching, supervision, and mentorship to registered nurses, healthcare assistants, and students. Collaborate with GPs and other health and social care professionals to develop care plans that promote independence. Drive clinical governance, audits, safeguarding, infection prevention, and adherence to policies while supporting service improvements. Operate with flexible, autonomous practice to maintain responsive community nursing services and staff development.
