Community Staff Nurse, Band 5 – Out of Hours Team

Deliver holistic nursing care to patients in their homes, care homes, and clinic settings within the Out of Hours community nursing locality. Carry out clinical assessments and implement and evaluate treatment plans across wound care, catheter care, continence care, and End of Life care. Promote self-care and independence, deliver evidence-based practice, and support admission avoidance. Work autonomously while collaborating within an integrated Place Based Teams care group. Cover the borough of Walsall from 18:00 to 08:00, ensuring high-quality care and positive patient experience. Provide guidance and support to junior and untrained staff within the team.
