Home Healthcare Care Coordinator
Based in St Helens, Merseyside, the Care Coordinator drives the planning and ongoing management of home care packages for clients across the region. You will schedule rotas for care teams, monitor service delivery against care plans, and ensure high-quality, person-centred care. Serve as the primary contact for clients, families, and healthcare professionals, coordinating with management to resolve issues quickly. Participate in multi-agency meetings with Integrated Care Boards (ICBs) and contribute to care planning to meet client needs. Support the out-of-hours on-call service; no hands-on care required, with travel across Merseyside using your own vehicle. Requirements include a Full UK Driving Licence, at least 12 months in care coordination, strong organisation, and clear communication.
