Registered Nursing Associate or Experienced Healthcare Assistant
Support patient assessment, care planning, and delivery under supervision, with scope based on registration (HCA or Nursing Associate). Perform vital signs monitoring, MUST screening, MUAC measurements, diabetic foot checks, wound care, and other minor procedures. Identify deterioration early and escalate appropriately; contribute to MDT discussions, care reviews, and referrals to district nursing, dietitians, and social care. Administer medications and injections, including flu and COVID vaccines, following practice protocols and training. Manage care home caseloads, schedule visits, and coordinate with care home staff to ensure timely care interventions. Support PCN projects and vaccination campaigns, participate in audits and quality improvement, and maintain accurate patient records.
