仕事内容
Provide comprehensive in-home nursing care to patients in their homes on a full-time schedule of four 10-hour shifts with rotating weekends.
Perform nursing assessments, medication reconciliation, safety evaluations, and implement standardized protocols.
Administer prescribed medications (including IV therapy), obtain blood samples, and manage urinary catheters as needed.
Educate patients and families on illness management, medications, nursing interventions, and the care plan.
Identify changes in patient condition, adjust care plans accordingly, and document all assessments and coordination activities.
Participate in on-call rotations and collaborate with the care team to deliver high-quality home-based care.
