The role
Provide equitable, value-based primary care to local Medicare populations within a neighborhood network.
Operate in a center-supported setting focused on outcomes, preventive medicine, and reducing hospital admissions.
Assess, diagnose, and manage patients; oversee the primary care team and coordinate care delivery.
Prescribe and manage pharmacologic therapies; maintain accurate EMR data in Canopy and Greenway.
Collaborate with regional and center leadership to meet health quality goals and partner with Practice Managers to optimize center operations.
Qualifications include MD or DO; board certification in Internal Medicine or Family Medicine (or eligible); active unrestricted state license; DEA license; US work authorization; and Spanish bilingual proficiency; geriatrics fellowship or experience preferred.
