Team Lead for Community Nursing
Provide dynamic leadership and management to the community nursing team to ensure delivery of high-quality, responsive, efficient care. Oversee all personnel, workforce planning, training and performance management with robust capacity planning and target setting. Develop and redesign adult care pathways, promote care closer to home, and expand multi-disciplinary and multi-agency collaboration. Accountable for day-to-day management, sickness absence, grievances and disciplinary matters, and patient complaints. Lead governance, safeguarding, risk management, and continuous improvement to ensure best practice and patient experience. Collaborate with Locality Clinical Managers and the Community Matron to support neighbourhood development and system transformation initiatives.
