The role
A Rehabilitation Case Manager is responsible for managing caseloads, conducting initial needs assessments and home visits, designing tailored rehabilitation programmes, collaborating with multidisciplinary teams, communicating with healthcare professionals and clients, and producing clinical reports and care plans. Candidates are expected to have relevant post-graduate clinical experience, be registered with HCPC, NMC, or GMC, and possess knowledge of complex injuries including ortho, neuro, MSK, or psychological issues. Strong clinical reasoning, report writing, and communication skills are essential, along with the ability to work independently and travel when necessary. The role involves remote case management, with some travel for home assessments, operating in a flexible, home-based environment within the South East area. Full training and mentoring are provided, and applications are accepted from employed or associate candidates.
