An exciting opportunity for a Band 6 Nurse to join the hospital-at-home team in Greater Manchester.
The role delivers safe, acute care, monitoring, and treatment in patients' homes through a multidisciplinary model.
Responsibilities include comprehensive health care needs assessments, reassessment as needed, and evaluating nursing interventions.
Plan, prioritise, implement, and evaluate care; contribute to multidisciplinary, patient-centered care coordination.
Facilitate active involvement of patients, relatives, and carers; support effective discharge and avoidance of unnecessary hospital admission.
