Clinical Pharmacist – Heywood

This position involves providing clinical medicines management and face-to-face medication reviews within a multi-disciplinary team across Heywood PCN. The post holder will manage medicines on transfer of care, support patients with long-term conditions, and contribute to quality improvement projects. Candidates must hold an undergraduate pharmacy degree, be registered with the General Pharmaceutical Council, and possess an independent prescribing qualification. The role requires prior general practice experience, strong clinical judgement, and excellent communication skills. It is a part-time, permanent role working four days per week, with responsibilities tailored to optimise patient care and operational efficiency.
