The role
Provide comprehensive primary care within a team-based, patient-centered medical home model to a diverse patient population.
Collaborate closely with specialists and health coaches to coordinate care and improve quality outcomes.
Engage in quality improvement initiatives, maintain compliance, and contribute to policy development for standard of care.
Support population health goals and financial sustainability through evidence-based clinical interventions.
Maintain NY medical licensure, board eligibility/certification in Family Practice or Internal Medicine, and proficiency with EHR.
Spanish language skills and a strong commitment to teamwork and ongoing professional development are preferred.
