Primary Nurse/Health Coach: Provides health coaching and case management services to members across the health continuum, collaborating with physicians and members to develop care plans and goals. Utilizes clinical and motivational interviewing skills to support self-management and lifestyle changes. Conducts assessments, identifies barriers, and coordinates care and services.
