Coach - Fair Futures

This full-time role provides home-based or community-based case management to up to 15 families, with visits at least twice a month to assess needs, reduce risk, and improve family health and wellness. Engage primary caregivers and youth to set goals, address barriers, and ensure progress toward academic, career, independent living, and life goals. Complete timely, accurate documentation of sessions and collateral notes in the Care4 database. Establish and maintain collaborative relationships with colleagues and community providers to connect families with resources and support ongoing communication for effective goal setting. Facilitate life skills training, college preparation, and leadership development through individual and group sessions. Conduct intake and initial assessments, obtain essential consent forms, perform safety assessments during home visits, and comply with reporting protocols while working both independently and as part of a multidisciplinary team.
