The role
Deliver high-quality outpatient primary care, including diagnosis, treatment, and preventive services.
Manage chronic conditions and coordinate care with specialists to optimize patient outcomes.
Develop and implement patient-centered treatment plans and ongoing disease management.
Prescribe medications, monitor effectiveness, and adjust plans as needed, with emphasis on safety and quality.
Maintain accurate medical records and ensure effective communication within the care team.
Participate in population health initiatives and ongoing medical education to continuously improve care.
