Le poste
Lead a District Nursing Team delivering holistic, evidence-based care to housebound patients in Warrington.
Manage a defined caseload with autonomous practice, conducting comprehensive nursing assessments and care planning.
Utilise prescribing skills and collaborate with professionals to support complex, continuing care packages.
Provide leadership, mentorship, and staff development, including supervision of pre- and post-registration students.
Participate in audits, critical appraisal and health needs assessments to drive service improvement.
Work flexibly, including weekend rotas, to ensure safe, patient-centred care across the district.
