Provides practical supports to individuals in community and facility-based employment settings.
Supports development and ongoing revision of the Individual Habilitation Service Plan through input and data.
Segments job duties into actionable steps to facilitate learning and independence.
Monitors performance, determines when support can be faded, and ensures work meets industry standards.
Maintains accurate records of coaching services, tracks progress, and documents barriers and actions.
Responds to crisis per policy, liaises with employers, and upholds agency vision and values while performing related duties.
