Care Integration Coordinator - Weekend Night Shift

Under the direction of the Supervisor or Manager, this role provides support to the Care Management team coordinating care for identified members/patients across the healthcare continuum.
Serves as a collaborative healthcare team member and performs essential clerical tasks to support ongoing assessment and management activities.
Focuses on weekend night shifts, ensuring timely data entry, scheduling, and effective care coordination.
Facilitates communication among providers, patients, and families to support comprehensive care plans.
Required qualifications include medical terminology knowledge and at least 1 year of healthcare experience.
Preferred qualifications include an associate's degree, CMA certification on hire, and experience with Mcg Care Guidelines, Touchwork, IDX, and Meditech.
